Please accept our humble obeisances. All glories to Guru Maharaja. All
glories to Srila Prabhupada and his sincere followers.
We would like to inform you that Guru Maharaja is planning to give
initiations on the last day of Bhisma Panchaka, 28th November, 2012 which
falls on Rasa Purnima. Devotees wishing to get initiated at that time are
requested to kindly go through the following guidelines:
Date of Initiation: 28th November 2012 (Wednesday)
Last date of registering your details with JPS Office (Email:
jpsoffice@pamho.net / Phone: 03472-245277): 10th November 2012 (Saturday).
Registration can be also done by coming to the office personally.
The last date of registration is 13th November for Rasa purnima initiations
2012.
First and Second initiations: Candidates need to write the philosophical
test either under the supervision of the temple authorities or at the JPS
Office. The temple authority may contact the JPS office for getting the
details of the exam, etc.
When a candidate wants to write the exam at the JPS office, a convenient
date for the exam should be proposed and fixed by the candidate in
consultation with the JPS office. These candidates should also attend an
oral interview at the JPS office. The prospective candidates for initiation
or ISKCON authorities are requested to be in touch with the JPS office for
any clarifications or for any further assistance. All devotees are requested
to co-operate with the JPS Office to ensure a smooth initiation event.
GUIDE LINES FOR FIRST INITIATION CANDIDATES:
The aspiring 1st initiation candidates have to send their personal and
spiritual details to the JPS Office <jpsoffice@pamho.net> It must include
the following information:
a. Legal name
b. Date of Birth / Age
c. Gender
d. Educational qualification
e. Marital status (married / single etc)
f. Nationality
g. Present address
h. Name of the temple / yatra / bhakti vrksa / namahatta, the candidate
belongs to
i. Name of the recommending authority
j. When did the candidate take shelter from HH Jayapataka Swami Maharaja and
who recommended the shelter
k. Which lila would you like your name from? For Example:
(Krsna lila/Gaura Lila/ Rama Lila /Nrsimha Lila)
All the above information should be sent in CAPITAL LETTERS. Information in
other formats would be rejected. All temple authorities are requested to
kindly see that you send the list of candidates as mentioned above.
2. Please download the following 1st initiation forms from
www.jayapatakaswamioffice.com and fill them out according to the guidelines given:
a. Cover sheet
This must be filled out in CAPITAL LETTERS. Other formats would be rejected.
b. Recommendation sheet
The recommendation sheet should be signed by the temple authority along with
the stamp of their official ISKCON branch.
c. Biodata (in English form it is attached to the recommendation sheet)
This page must be filled out in CAPITAL LETTERS. Please do not overwrite in
any of the sections.
d. Comprehensive Check list. (to be left blank and will be filled in at the
time of oral interview)
e. Practical questionnaire.
f. Essay question: Why do you want to take initiation from Guru Maharaja?
The candidate should answer all the questions mentioned here. Illiterate
candidates should answer orally, whereas others can write their answers on
this sheet.
g. Initiation oath
The signature of the candidate along with the two first initiated candidates
witness signatures and phone numbers are required.
h. One passport size photo.
All the above forms should be filled out properly and should be sent to our
office by courier (DTDC courier is preferred since its branch is available
here in Mayapur) or brought with you to the JPS Office.
The office will verify all your documents .
Resident devotees qualified to take initiation can contact the JPS Office to
fix the date to write the exam and attend the oral interview.
They can begin to contact the JPS office right away to finish off the
initiation procedures at the earliest.
For 2nd initiation candidates:

b. Age
c. Educational qualification

belongs to.
h. Name of the recommending authority
i. Date of first initiation from HH Jayapataka Swami
2. Please download the following 2nd initiation forms from
www.jayapatakaswamioffice.com. Fill out all the forms carefully using the guidelines
given below:
a. Cover sheet
This document must be filled out in CAPITAL LETTERS. Other formats
will be rejected.
b. Recommendation sheet
The recommendation sheet should be signed by the temple authority along with
the stamp of the official ISKCON branch that is giving the recommendation.
d. Bio-data (in English form it is attached to the recommendation sheet)
This page must be filled out in CAPITAL LETTERS. Please do not
overwrite in any of the sections.
e. Signature from three Brahmanas
f. Practical check list (to be left blank and will be filled in at the time
of oral interview)
e. Philosophical exam
If you are qualified as the holder of a ISKCON Bhakti-Shastri Certificate
and would like to be exempted from writing this philosophical test, then you
must enclose a copy of the Bhakti Shastri Certificate.
f. Practical questionnaire: Candidate has to fill it up correctly.
g. Essay “Why I want to take 2nd initiation? ”
The prospective candidate has to write an essay on the above subject
mentioning the clear purpose and objective of taking second initiation.
Illiterate candidates also should get it written by someone, but the
initiation candidate’s objectives and purpose of taking the 2nd initiation
must be mentioned.
3. Oral interview: All candidates are interviewed by the JPS Office if the
candidate passes the written examination. If candidates are interviewed by a
competent body, a certificate or letter should be provided to this effect to
the JPS office.
Forms are available in English, Bengali, Hindi and Spanish languages at the
JPS office website www.jayapatakaswamioffice.com
All the above forms should be filled out properly and should be sent to our
office by courier (DTDC courier is preferred since its branch is available
here in Mayapur) or brought with you to the JPS Office.
The office will verify all your documents.
If any devotee or candidate is already in Mayapur, then their interviews and
examination can take place now.
It is IMPORTANT that the guidelines in the CHECK LIST must be followed
strictly by all candidates.
They are expected to know by rote all prayers & mantras and to have read all
the recommend books, the prescribed number of times.
The candidate or ISKCON authority is requested to kindly be in touch with
the JPS office for any clarifications or for any further assistance.
