We would like to inform that His Holiness Jayapataka Swami

Maharaja is planning to give initiations on the most auspicious day

of Sri Gaura Purnima (12th March 2017). Devotees wishing to take

Harinam and Gayatri (Brahmin) initiation from H.H. Jayapataka Swami

Maharaja on this occasion, are requested to carefully go through

the following information.

**Date of Initiation: 12th March, 2017

(Sunday)**

Devotees wishing to take initiation on this occasion; have

to register with JPS office by no later than 28th February

2017, by submitting your details through the online

registration form (given at the end). For those who do not have

internet access, may register by calling us on: (+91)03472- 245277

during the office hours. You may also send a mail to jpsofficedatabase@gmail.com to register your name.

Working hours at JPS office: 10 am -5.30 pm (Mon-Fri); (Sunday

closed, Saturday 10 am – 2 pm).

Please note that the completion of the ‘ISKCON Disciples Course

(IDC)’ is mandatory to take initiation with effect from Janmastami

2015 as per the GBC mandate. For detailed information please visit

Aspiring initiation candidates must show the original copy of the

ISKCON Disciples Course completion certificate during the interview

and submit a xerox copy of the same along with other initiation

The candidate or concerned ISKCON temple/yatra/Bhakti Vriksha

authority are requested to kindly be in touch with the JPS office

office before fixing your initiation exam/interview appointment

time as well as to clarify your doubts, if any, in regard to the

As you know that Sri Jagannatha Rathyatra festival is very close. Like the previous years, His Holiness Jayapataka Swami Guru Maharaja will grace the festival of chariot at Kolkata. After the return rathyatra, there will be initiation ceremony at Sridham Mayapur on 19th of July on the auspicious Vyasa purnima tithi. Registration for Radhastami Initiation – 2015

Dear Devotees,

We would like to inform you that His Holiness Jayapataka Swami Maharaja is planning to give Harinam and Brahminical initiations on the most auspicious day of Radhastami (21st September 2015) at Sri Mayapur dham. Devotees wishing to take initiation from H.H. Jayapataka Swami Maharaja on this occasion, are requested to please go through the following information.
**Date of Initiation: September 21, 2015 (Monday)**

Devotees wishing to take initiation on Radhastami, 21st September 2015; have to register with JPS office by no later than 12th September 2015, by submitting your details through the online registration form or sending an email to: jayapatakaswamiofficedatabase@gmail.com; or call on: (+91)03472- 245277 during the office hours. Working hours at JPS office: 10 am -5.30 pm (Sunday closed, Saturday 10 am – 2 pm).

The devotees who want to take initiation have to fill out the required personal data in the form given at the end of this mail and then click submit. Then your details come to us and you are registered with JPS Office.

Harinam and Brahmin initiation candidates need to write the philosophical test. This test can be written either under the supervision of their concerned ISKCON Temple/Bhakti Vriksha authorities or at JPS office in Mayapur. The concerned authority should send the evaluated answer sheet with their evaluation remarks to the JPS office along with the filled out initiation forms.

Initiation forms are available in the JPS office website http://www.jayapatakaswamisoffice.info/?page_id=3. The candidates can take a print out of the desired form and has to fill it out. Candidates who wish to write the philosophical exam at JPS office have to fix up a mutually convenient date for the test.

JPS office will conduct written exams and interviews till 18th September 2015 (Friday)
Please note that the written exam format for Brahmin initiation candidates consists of descriptive questions and Multiple Choice Questions (MCQ) based on the topics mentioned in the ‘2nd initiation philosophical test’ sheet which is available in the following link http://www.jayapatakaswamioffice.info/?page_id=3. ISKCON Bhakti Shastri qualified persons can opt for exemption of writing the descriptive test. In such case, the candidate has to submit the Bhakti Shastri certificate during the interview at JPS office. Answering the multiple choice test is compulsory for all the second initiation candidates irrespective of any Bhakti Shastri qualification.

All 1st and 2nd initiation candidates have to attend an oral interview at JPS office.
Note: As per the ISKCON GBC requirements the test has 2 questions regarding Srila Prabhupada’s position in ISKCON and ISKCON’s lines of authority. To answer these questions correctly, please go through the ‘GBC Resolution 303/2013: “The Position of Srila Prabhupada’ and ‘Harmonizing ISKCON’s Lines of Authority”. Steps to be followed by the candidate:
1. Check the eligibility for the initiation with local ISKCON authority. If needed, you may call us to clarify any doubts.

2. Fill out the online form, submit it to the JPS office to register your name for initiation. (All the checklist and forms are available in our website http://www.jayapatakaswamioffice.info/?page_id=3)

3. Please contact Mani Gopal das at JPS Office to write any written test and to fix the interview date by 10th September 2015 at the latest.

4. Please submit all the required documents to the JPS office with proper approvals and recommendation. You may send your papers either through email or by personal visit to the JPS Office.

Instruction for the 1st (Harinam) initiation candidates:
Please download the following 1st initiation forms and fill them out in CAPITAL LETTERS only and avoid overwriting.

a. Cover sheet
b. Recommendation sheet (The recommendation sheet should be duly signed by the concerned temple authority along with the office stamp of that particular branch of ISKCON).
c. Biodata (in the English form this can be found in the recommendation sheet as a separate part it).
d. Comprehensive Check list. (This should be filled out in the JPS Office at the time of oral interview)
e. Personal questionnaire: Fill out your personal information as applicable to you.
f. Essay: You should write an essay “Why do you want to take initiation from Jayapataka Swami?” answering the questions asked in this sheet.
g. Initiation Oath: The signature of the candidate along with the two witness’ signatures.
h. One passport size photo.

On successful completion of the written exam of Harinam initiation, candidates will be interviewed by the JPS office and the information with the evaluation of JPS office will be sent to HH Jayapataka Swami Maharaja.

Instruction for the 2nd (Brahmin) initiation candidates:

f. Essay: You should write an essay “Why I want to take 2nd initiation?” answering the questions asked in this sheet.

(Candidate has to write an essay on the above subject mentioning the clear purpose and objective of taking second initiation. Illiterate candidates also should get it written by someone, but the candidate’s personal objectives and purpose of taking the 2nd initiation must be mentioned.)
g. One passport size photo.

On successful completion of the written (descriptive and MCQ) exam of Brahmin initiation the candidate will be interviewed by the JPS office and the information with evaluation of JPS office will be sent to HH Jayapataka Swami Maharaja.

The candidates are expected to know by heart all prayers & mantras and to have read all the books mentioned the prescribed number of times.

*** Tulasi chanting beads for the Harinam initiation candidates are issued by JPS office. Therefore please do not bring your own chanting beads. ***
