Please accept our humble obeisances. All glories to Srila Prabhupada and his sincere followers. 

This is for your kind information that His Holiness Jayapataka Swami Maharaja will give initiation during the upcoming Gaura Purnima festival. Due to Guru Maharaja’s tight travel schedule, this time international (devotees from outside of India) and national (devotees from the temples/yatras from other states of India than West Bengal) candidates would be considered for initiation. All local (West Bengal) devotees are requested to contact JPS office after Gaura Purnima for the next initiation schedules.     

The tentative date of initiation is any day between 6th to 8th March, 2020. Please note that the date could be changed, to a day before or after the given date, for any unforseen circumstances. 

Last date of submitting names to JPS office: 25th Feb, 2020
To register candidate’s name to JPS office for Harinam or Brahmin initiation, please go the following link and submit the details. After registering details, the candidates/the concerned authority must send his/her documents i.e. initiation form, recommendation letter, IDC certificate and bhakti sastri certificate (for 2nd initiation) to jpsofficedatabase@gmail.com.
JPS office first will check candidate’s shelter record into office database. In case, if the candidate has taken shelter before but was not recorded with the office, they will be asked to send the scanned shelter form duly signed by the temple authority. Without shelter record, no one will be considered for current initiation.     
JPS Office strictly maintains the initiation standard set by GBC and HH Jayapataka Swami. Candidate should meet all the checklist requirements (prayers, mantras, basic philosophy and book study) to apply for initiation.  To know detailed eligibility criteria, for applying Harinam or Brahminical initiation, please go through the link attentively.

Submission of ISKCON Disciples Course (IDC) certificate is mandatory for all candidates. Submission of Bhakti Sastri certificate along with IDC certificate is mandatory for Brahminical initiation candidates.

We would like to inform you that His Holiness Jayapataka Swami Maharaja is planning to give Harinam and Brahminical initiations on the most auspicious day of Nrsimha Caturdasi (3rd May 2015). Devotees wishing to take initiation from HH Jayapataka Swami Maharaja on this occasion, are requested to please go through the following information.

**Date of Initiation: May 3rd, 2015 (Sunday)**

Devotees, wishing to take initiation on this occasion, have to register with JPS office by NO later than 20th April 2015. For registration the candidates need to submit their details through the online registration form (see below) or sending an email with the information mentioned in the below form. Please write to the JPS Office initiation coordinator at: jpsofficedatabase@gmail.com; or call on: (+91)03472- 245277 during the office hours. Working hours at JPS office: 10 am – 5.30 pm (Sunday closed, Saturday 10 am – 2 pm).

The devotees who want to take initiation have to fill out the required personal data in the form given at the end of this mail and then click submit. Then your details come to us and you are registered with JPS Office.

Harinam and Brahmin initiation candidates need to write the philosophical test. This test can be written either under the supervision of their concerned ISKCON Temple/Bhakti Vriksha authorities or at JPS office in Mayapur. If the candidate writes the test at the adjacent temple/BV under the supervision of the concerned authority, then the authority should send the evaluated answer sheet with their evaluation remarks to the JPS office along with the filled out initiation forms.

Initiation forms are available in various languages in the JPS office website http://www.jayapatakaswamioffice.com/?page_id=3. The candidates can take a print out of the desired form and fill it out. Candidates, who wish to write the philosophical test at JPS office, have to fix up a mutually convenient date for the test.

JPS office will conduct written exams and interviews till April 30th 2015 (Thursday).

Please note that the written exam format for Brahmin initiation candidates consists of descriptive questions and Multiple Choice Questions (MCQ) based on the topics mentioned in the ‘2nd initiation philosophical test’ sheet which is available in the following link http://www.jayapatakaswamioffice.com/?page_id=3. ISKCON Bhakti Shastri qualified persons can opt for exemption of writing the descriptive test. In such case, the candidate has to submit a copy of the Bhakti Shastri certificate during the interview at JPS office. Answering the multiple choice test is compulsory for all the candidates irrespective of any Bhakti Shastri qualification. Please note that, the oral interview is compulsory for all 1st and 2nd initiation candidates.

The candidates or concerned ISKCON temple/Yatra/BV authorities are requested, regarding Nrisimha Caturdasi initiation, to contact the JPS office by 20th April at the latest. We advise not to book travel tickets or make arrangements without contacting the JPS office for the initiation purpose as JPS Office clarifies any doubts on this issue about the eligibility of the candidates and other procedures.

All are requested to cooperate with the JPS office to ensure a smooth initiation event.

Steps to be followed by the candidate:

1. Check the eligibility for the initiation with local ISKCON authority. If needed, you may call us to clarify any doubts.

2. Fill out the online form, submit it to the JPS office to register yourself for initiation. (All the checklist and forms are available in our website http://www.jayapatakaswamioffice.com/?page_id=3)

3. Please contact Mani Gopal das (initiation coordinator) at JPS office to fix the date for written test and oral interview by 20th April 2015 at the latest.

4. Please submit all the required documents to the JPS office with proper approvals and recommendation. You may send your papers either through email or by personal visit to the JPS Office.
Instruction for the 1st (Harinam) initiation candidates:

Please download the following 1st initiation forms and fill them out in CAPITAL LETTERS only and avoid overwriting.

a. Cover sheet

b. Recommendation sheet (The recommendation sheet should be duly signed by the concerned temple authority along with the office stamp of that particular branch of ISKCON).

c. Biodata (in the English form this can be found in the recommendation sheet as a separate part of it).

d. Comprehensive Check list. (This should be filled out in the JPS Office at the time of oral interview)

e. Personal questionnaire: Fill out your personal information as applicable to you.

f. Essay: you should write an essay “why do you want to take initiation from Jayapataka Swami?” answering the questions asked in this sheet.

g. Initiation Oath: The signature of the candidate along with the two witness’ signatures.

h. One passport size photo.

On successful completion of the written exam of Harinam initiation, candidates will be interviewed by the JPS office and the information with the evaluation of JPS office will be sent to HH Jayapataka Swami Maharaja.

Instruction for the 2nd (Brahmin) initiation candidates:

Please download the following 1st initiation forms and fill them out in CAPITAL LETTERS only and avoid overwriting.

a. Cover sheet

b. Recommendation sheet (The recommendation sheet should be duly signed by the concerned temple authority along with the office stamp of that particular branch of ISKCON).

c. Biodata (in the English form this can be found in the recommendation sheet as a separate part of it).

d. Comprehensive Check list (This should be filled out in the JPS Office at the time of oral interview)

e. Personal questionnaire: Fill out your personal information as applicable to you.

f. Essay: You should write an essay “why I want to take 2nd initiation?” answering the questions asked in this sheet.
(Candidate has to write an essay on the above subject mentioning the purpose and objective of taking second initiation. Illiterate candidates also should get it written by somebody else, but the candidate’s personal objectives and purpose of taking the 2nd initiation must be mentioned.)

g. One passport size photo.

On successful completion of the written (descriptive and MCQ) exam of Brahmin initiation the candidate will be interviewed by the JPS office and the information with evaluation of JPS office will be sent to HH Jayapataka Swami Maharaja.

The candidates are expected to know by heart all prayers & mantras and to have read all the books mentioned the prescribed number of times.

*** Tulasi chanting beads for the Harinam initiation candidates are issued by JPS office. Therefore we strongly suggest please do not bring your own chanting beads. ***
