glories to Srila Prabhupada and his sincere followers.
Guru Maharaja is planning to give initiations at Mayapur during Gaura
Purnima which falls on 8th March this year.
If any of you are planning to take initiation at this time then kindly do
the following:
For first initiation candidates

1.Please immediately send your:
a. Legal name
b. Age
c. Gender
d. Educational qualification
e. Occupation / devotional service
f. Marital status (married / single etc)
g. Nationality
h. Which lila you would like you like your name from? For example
Krsna/Rama/Nrsimha/Visnu/Gaura
The deadline to receive the above info is 20 Feb 2012.
All the above information shold be sent in CAPITAL LETTERS. Information in
other formats would be rejected.
All temple authorities, kindly see that you send the list of candidates as
mentioned above.
2. Please download the following 1st initiation forms from
www.jayapatakaswamioffice.com
a. Cover sheet
This should only be filled in CAPITAL LETTERS. Other formats would be
rejected.
b. Recommendation sheet with Bio-data. The recommendation sheet should be
signed by the temple authority along with the stamp of their official ISKCON
branch.
c. Check list – the practical part should be left blank as the oral
interview will be conducted here in the office but the philosophical exam
should be written in front of the temple authority or in front of a temple
authorized devotee and it should be a closed book examination. You can
prepare in advance before writing the exam. Once the exam is written the
temple authority should correct the paper and calculate the percentage marks
obtained. Each question carries 5 marks and the marks get reduced to 4, 3,
2, 1, 0 based on the length and correctness of the answer. Add the total and
deduce the percentage mark for each candidate.
d. Practical questionnaire
e. Essay why you want to take initiation from Guru Maharaja.
f. Initiation oath
This should only be filled in CAPITAL LETTERS. Other formats would be
rejected.
g. Two passport size photos
3. Oral interview
All the above forms should be filled in and brought with you to Mayapur.
The oral interview will be conducted between 29 Feb 2012 and 5 March 2012
at JPS Office which is loacted at 105, Chakra Building or Long Building. No
interviews will be accepted after these dates. JPS Office hours are from 10
am to 5.30pm.
Kindly book your tickets and rooms in advance so you can be present here
within these dates. If any devotee or candidate is already in Mayapur then
their interviews and examination can take place now.
For 2nd initiation candidates

2. Please download the following 2nd initiation forms from

This should only be filled in CAPITAL LETTERS. Other formats would be
rejected.
b. Recommendation sheet with bio-data. The recommendation sheet should be
duly signed by the temple authority with the official stamp of their ISKCON
branch.
c. Signature from three brahmanas
d. Practical check list (to be left blank and will be filled in at the time
of oral interview)
e. Philosophical exam
You can prepare in advance for this exam but the answers should be
written closed book in front of the temple authority or a temple authorized
representative.The exam should be corrected by the temple authority and
percentage marks deduced. If you have done an ISKCON Bhakti-sastri
examination then there is no need to write this exam but you need to provide
a xerox copy of your certificate.

that everything is organized nicely so that it doesn’t become a strain on
Guru Maharaja’s health.
It is IMPORTANT that the CHECK LIST is followed strictly for all candidates.
They are expected to know by rote all prayers & mantras and to have read all
the books mentioned, the prescribed number of times.
